Insurance & Payment Information

Dr. Miller is an in-network provider with Blue Cross Blue Shield and CBHA (MedCost), and participates out of network with several other insurance panels. Please contact your insurance carrier to:

• Verify that Dr. Melissa Miller is covered by your insurance
• Obtain information about your co-payment
• Obtain information about your deductible
• Determine if your deductible has been met for the current year
• Obtain authorization number for outpatient mental health services

Please contact your insurance company to get specific information on your out-of-network benefits, or call Dr. Miller for details.

For those who do not wish to use insurance, Dr. Miller’s fees are:

• $150 Initial Evaluation
• $125 45-50 Minute Session
• $75 No Show/Late Cancellation Fee

Payment is due at the time of service. Dr. Miller accepts cash, checks, or credit card payments.
