

Q: Why do I have to have a psychological evaluation?
The surgeon referring you for evaluation wants to make sure that you are informed and emotionally prepared for the surgery and the dramatic changes that you will experience afterwards. Many insurance companies also require this examination prior to authorizing coverage for gastric bypass surgery.
Q: What will the evaluation involve?
The psychological evaluation consists of a paper-and-pencil psychological test and an interview. The tests will help the psychologist learn a lot about you as a person in a brief time period. During the interview you will be asked about many aspects of your life, including information about your past, your present situation, and your future goals after the surgery.
Q: Can I fail the test?
The test that you will be asked to complete is not a pass-fail test and there is no preparation necessary prior to taking the test. The testing can help the psychologist identify your particular strengths and assist in identifying any areas that need to be addressed to prepare you for surgery and maximize your potential for long-term success with the necessary changes that will follow.
Q: How long will the evaluation take?
The evaluation can usually be completed in an hour and a half. You will first complete the testing, which can take from 30-45 minutes; then you will be seen by the psychologist for an interview that usually takes 45-50 minutes.
Q: Will I have to see the psychologist more than once?
This evaluation can usually be completed in one visit. In addition, the psychologist may make recommendations for further psychological or psychiatric interventions, but only if indicated.
If you would like to see the psychologist for follow up therapy, you are welcome to discuss this with the psychologist or call and schedule an appointment.
Q Who will get the psychologist’s report?
Our office will send a copy of your report to your surgeon with your written permission. Your insurance company may receive a copy of the evaluation from your surgeon if this is required for authorization for surgery. You may request a copy be sent to your Primary Care Physician.
Q: Will my insurance cover the cost of the evaluation?
The psychological evaluation is often covered by the mental health portion of your insurance or by the medical portion of your insurance. Some plans exclude Bariatric evaluations. Please contact your insurance company to determine your coverage. The $35.00 materials fee is not covered by insurance. If your insurance requires pre-authorization, you will need to obtain pre-authorization for an initial assessment (90801) and psychological testing (90100) ahead of time and call our office with the authorization number.
Q: How much does the evaluation cost?
The evaluation, includes both a charge for the clinical interview and the psychological testing. Please call the office for current fees for these services. In addition, there is a $35 materials fee that is not covered by insurance. Payment in full is required at the time of the evaluation unless you have an authorization number from your insurance company, in which case you will pay your co-payments (you will pay a co-payment for the initial assessment and a co-payment for the psychological testing- in rare instances insurance companies do not require 2 co-payments in which case you will be reimbursed) and the $35 materials fee. If you do not have an authorization number, you will be given information necessary to file your insurance claim yourself.
Q: What if I have to cancel?
Cancellations must be made at least 24 hours before your appointment time. Any cancellations made with less than 24 hours notice will be subject to a $40. charge.
